Appointments

Appointments are an important part of your treatment, as time has been reserved specifically for you. Please arrive at least 15 minutes prior to your appointment.

Cancellation and No-Show Policy

Patients who cannot keep their appointments are asked to provide advanced notice of cancellation and the need to reschedule.

Due to an increase in “no-show” appointments, the following policy has been created:

A "no-show" is defined as a scheduled appointment that a patient does not keep or an appointment that is rescheduled without advanced notice.  When this occurs, patients will be charged a NO SHOW fee as follows:

o   Routine Appointments: A $25 fee will be charged if the appointment is not canceled/rescheduled within 24 hours of the scheduled appointment

o   Procedures and Ultrasounds:  A $50 fee will be charged if the appointment is not canceled/rescheduled within 48 hours of the scheduled procedure or ultrasound (due to the increased duration of these appointments) 

Until fees for missed appointments are paid, patients will not be able to schedule future appointments.

Financial

We never want costs to stand in the way of quality treatment, which is why we are dedicated to providing our patients with care that is both effective and affordable. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We offer a variety of payment options to meet your needs. We accept Visa, MasterCard, and Discover.

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.

When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not been paid within a reasonable timeframe. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.

Medical Records

By law, every person is entitled to copies of their medical records. This includes lab and test results and doctor's notes.

Release of Medical Records

We will be glad to provide copies of your medical records; however, depending on the number of pages requested, you may be charged a fee for printing. Most times, you only need one or two pages of your records and not your complete medical history with our office. All requests for medical records must be submitted in writing. Please allow 3 to 5 days to process your request.

Medical Records from a Referring Physician

When your doctor refers you to our practice, we typically request a copy of your medical records, including recent lab work and imaging tests. If your doctor does not provide this information to us by the time of your first visit, you may bring these documents with you.

Prescription Refills

We ask that you bring all prescription medication(s) with you to your appointment, even if it was prescribed by another physician. Please request all refills of continuous medicines during your appointment.

In the event of your prescription expiring prior to your next appointment, contact us. Generally, we can request a prescription be refilled at your pharmacy within 24 hours.
